We are located at the corner of 26th & Spruce Streets in Boulder.

There is plenty of free parking available.

We are open weekdaysfrom 8AM to 5PM; on Saturdays we are open from 9AM to 1PM.

We close from 1-2PM for lunch. The switchboard is open from 8AM to 5PM.

After hours, dial 303-449-3594 and press "0" and leave a message for the doctor on call. When you hang up, they will be paged. They will listen to your message and then call you back. If you do not recieve a call back in 15 minutes, please call again. Rember that cell phones can often transmit poorly.

VOICE MAIL: If the office staff is not available to take your call, you will be connected to their voice mail. Please leave a detailed message, and clearly state your phone number. If you do not receive a return call within a reasonable time, please call again. Out of state calls will be returned collect.

OFFICE FAX: 303-449-3112 We welcome messages from you by fax, and can fax back to you if you wish, if your fax is confidential. Please call first if a fax needs urgent attention.


 

If you have a medical emergency don't delay - call 911 first.

If you have an urgent medical need and the office is closed, dial 303-449-3594.  You will get instructions for how to leave a message for the doctor on call; when you hang up they will be paged. 

Please have your insurance and pharmacy information available when you call.

If you do not receive a return call within 20 minutes, please call back and page the doctor again. 


If you are ill during the week, and want to be seen, please call in the morning; we reserve time slots for urgent visits. If you don't mind waiting we will "work you in" during the day.

If you are not sure whether you need to come in, please discuss your problem with our office staff. We have trained them to help you decide what to do.

Always call if you are sick or hurt before you drop in just in case the office is unexpectedly closed.

Payment is expected at the time of service. If you have a deductible, you will need to present a credit card for us to keep on file. 

If we must bill you, there is a $35 late fee if payment is not received within 30 days.  Accounts older than 90 days will be sent to our collection agency. 


APPOINTMENTS & CANCELLATIONS  return to top of page

We respect your time and strive to stay on schedule. To provide you with our full attention, we do not overbook the doctor's schedule. Please help us by being on time to your appointment.

We have recently initated an "open access" policy.  That means that we blcok out spaces for patients who would like a "same day" appointment.

Call ahead if you know that you will be late.

If you arrive late, please understand that it may be necessary to reschedule your appointment, or you may need to wait for a "work in" spot.

If you must cancel your visit, please call at least 2 hours ahead of time or we will have to charge you a $35 "no-show" fee.

We estimate the time needed for your visit based on what you tell us about your problem, so be sure to tell our staff about all of your medical problems.

If you have time constraints yourself, please let the receptionist know when you arrive.

Your health insurance card is just like a credit card. Please bring your insurance card and co-payment, if you have one, with you to every appointment.


MANAGED CARE return to top of page

Please familiarize yourself with the rules and covered benefits of your insurance policy.

We do not know what is covered under your policy. Ultimately it is your responsibility to follow the rules of your insurance plan if you want them to pay.

Managed care plans require a specific laboratory or hospital for diagnostic tests, so you must show us your insurance card at each visit.    For other services, such as X-ray services, it may not be possible for us to know the least expensive  facility for you particuiar plan – to find the correct and expensive service, please call your insurance company.

If the wrong lab or hospital is used, we may be penalized, or you may have to pay for the testing.  The same is true for referrals to specialists or for other care.

  1. We must refer you to specialists within your insurance plan.
  2. If you see a specialist without getting the proper referral prior to the visit, you may have to pay for that visit.
  3. We cannot obtain a referral for you after the fact.
  4. We cannot make exceptions without suffering financial penalties -- your insurance company makes these rules, not us!

Some insurances have a "wellness" benefit for routine exams and screening labwork, while others do not cover these services.  We have no way of knowing what you particular plan coversPlease find out what your insurance covers before you come in.

You may want services that are not covered by your insurance, such as travel shots, or screening for certain diseases. We will inform you in writing of the cost of non-covered services in advance.

Please do not ask us to alter the diagnosis or type of service in order to get your insurance to pay for non-covered services -- this is illegal and unethical.
